 Job Description: St.George’s University Hospital is one of the UK’s largest teaching hospital with more than 1000 beds and has a well established  Plastic & Reconstructive unit. Being appointed as a Senior doctor in this speciality I work independently as well as with the Lead Clinician Prof. B.Powell who is well know for his work on Melanoma!

I do independent clinics in  St.John’s & St.George’s Hospital, London and in Royal Surrey County Hospital, Guilford beside doing combined melanoma & PRD clinics.

My operative commitments include three sessions Tuesday list with Prof. Powell or independently involving mainly melanoma surgeries eg. SNBs, various lymph nodes dissections including neck dissection & reconstructive surgeries for skin cancers. I have half day Friday independent list of my own involving aesthetic procedures eg. Breast implants, reduction, mastopexy, abdominoplasty etc. I also perform these aesthetic / uplift procedures on my Saturdays  Ad hoc full day lists in St. Georges Hospital.  I do minor operative sessions every Wednesday at the off sites hospitals in Surrey.

Beside these regular commitments  I also do independent Adhoc clinics & minor op lists on Saturdays in Royal Surrey County hospital and Internal consultant Locum in St. Georges Hospital when a skin sub speciality consultant is away.

There were 7 consultants, 2 Associate specialists, 7 registrars and 7 SHOs.

I as associate specialist had my own independent OPDS and both elective

and Trauma lists. I did 2 adult and 1 paediatric elective operative lists, 1 LADC list, 1 Laser list and 1 or 2 trauma lists involving hand trauma & burns. In case of elective lists the cases ranged from aesthetic procedure like abdominoplasty, breast reductions/ augmentations/mastopexy, otoplasty, syndactyly/polydactyly to insertion of tissue expander, liposuction, breast reconstruction, trapezectomies etc.

I was promoted to the position of Associate Specialist by a panel of Newcastle upon Tyne Hospitals NHS trust after a year or so of working at Senior staff grade doctor level, on recommendation of consultants colleagues & in view of ability to work at independent level with confidence.

Job Description: Northern General Hospital is 1,000 beded teaching hospital, linked to both Sheffield Childern’s hospital and the Royal Hallamshire hospital. The Plastic surgery dept. has 28 inpatient beds, 6 day care beds and 10 beds in the burn unit, beside having 11 beds in the Sheffield Childern’s hospital.

          The dept. of Plastic surgery was staffed by 7 consultants, 6 SpRs,  Senior Clinical Fellows, a Research Fellow and 4 senior & 1 pre-registration house officers. This post was structured in a pattern similar to that of a SpR Training programme  during which time I received training in all aspects of Plastic surgery with special emphasis on Hand Surgery.There were regular operating sessions under the guidance of consultants as well as dealing with the emergencies. The on-call rota was one in six with the SpRs.

Jobs Description:

           St.Vincent University Hospital houses the National liver transplant unit beside having a well organised Plastic & Reconstructive unit. The unit is staffed by two consultants, two registrars, a SHO and an intern. There are three elective theatre lists each week in above hospital, beside separate lists in St.Vincent private hospital, Blackrock Clinic & Mount Carmel Hospital.

          Our Lady’s Hospital for Sick Children houses the National paediatric unit for  plastic surgery and burns. Four consultants supervise the unit, which is staffed by a Registrar and a SHO. There were four elective theatre lists each week beside burn and trauma lists.

          St. James’s Hospital houses the National adult unit for plastic surgery & burns, besides having a well-equipped laser unit. Four consultants, three registrars, three SHOs & two interns staff the unit. There are seven elective theatre lists each week beside separate burns and trauma lists. 

          Cork University Hospital is the Regional plastic surgery & burns referral centre for the south of Ireland, which is staffed by two consultants, four registrars, two SHOs & an intern.                                           RESEARCH EXPERIENCE 

 

        I am interested in basic research which have clinical implications or are clinically oriented as can be noticed by my above mentioned presentations and publications, for example :-

       

       I did the basic cadaver study followed by it’s clinical implication in active mobilisation of Flexor tendons in Zone V .

 

       I was the first surgeon in St. James’s Hospital, Dublin,  to use  Mitek Mini anchors for the repair of Central slip of extensor tendons & the Open Mallet followed by clinical review of the cases done using that technique.

                         TEACHING & TRAINING EXPERIENCE

 

 

   In my present position as Associate Specialist in St. George’s University Hospital, London, Iam involved in hands on teaching of Specialist Registrars plus Medical students of St. Georges University.

     I have been actively involved in teaching medical students, junior colleagues and student nurses while working as Senior Medical demonstrator and subsequently in all my clinical jobs especially as Assistant Professor General Surgery in Aga Khan University Hospital Karachi, Pakistan.

      Being Assistant Professor Surgery I was actively involved in teaching medical students and surgical residents. Moreover I conducted Journals reviews and weekly surgical grand round & speciality grand round assessing medical students & residents presentations.     

     Previously as Medical Demonstrator in Anatomy & lastly as Assistant Professor in Basic Biological Sciences I taught regional anatomy to medical students, took tutorials for medical student/ residents.

      I was regularly involved in preparing MCQs both for undergraduates & postgraduates besides invigilating during exams.

 

 

 

 

                       MANAGEMENT EXPERIENCE

 

      

 Heading the Section of Plastic Surgery I ensured this speciality is appropriately managed in Aga Khan University Hospital. I regularly attended the Surgery Faculty meetings and was member of various committees eg. Standard of Care committee for the Operating theatre and AKUH Journal of Surgery “Cutting Edge” (A Founding member of Editorial committee).

 

     Previously working in the capacity of Senior Registrar I was involved in smooth running of the unit liaising with my consultants colleagues & with input from my junior colleagues, besides co-operating with nursing staff, colleagues from other speciality and administration. As an Associate Specialist I was called upon to delegate duties appropriate to experience of my junior colleagues and myself undertaking tasks, which require judgement and decision-making.

   

      I have been rota co-ordinator and arranging operative lists and outpatients, while working as Administrative Registrar. As Associate Specialist I attended the Plastic Surgery unit business meeting regularly along with consultant colleagues.

   PROCEDURES PERFORMED

 

General Techniques:-

 

Wound management: including EUA, toilet/ irrigation, debridement of extensive complex wounds, necrotizing fasciitis, high pressure injection injury, abscess drainage, Vacuum dressing, haematoma/seroma evacuation and various techniques of wound closure including primary /delayed closure, tissue expansion, using skin graft & various types of flaps.

Scar management: including simple or serial excision with direct closure or z-plasty /w-plasty, scar revision using dermo-fat graft or filler and scar excision with graft or flap reconstruction.

Graft harvest: including split thickness/ full thickness skin graft, nail bed graft, buccal mucosal/ foreskin graft, cartilage/ bone graft, tendon/ nerve/ vein graft and composite graft harvesting.  

Pressure Sores management: Debridement of Ischial, Sacral, Trochanteric & other pressure sores with application of Vac dressing and reconstruction using skin graft or  various flaps including Gluteal Flaps (rotation fasciocutaneous &  V-Y myocutaneous), Tensor fascia lata flap.

Vascular malformations: including excision of excess boggy skin after involution of strawberry haemangioma, single stage of smaller lesions and serial excision or debulking of facial haemangiomas.

Laser use: Co2 Laser for cutaneous lesion including melanoma mets, warts ablation, PDL for port wine stain & NdYag for spider nevi and Laser resurfacing.

 

Skin & Oncology:-

 

Specific Skin Conditions Management:

Hidradenitis suppurativa:  Limited excision of affected area with primary/ delayed closure, Radical excision of hair bearing area of axilla, groin/ thigh and reconstruction with Split skin graft or local/ regional flap including Posterior Arm Flap.

Hyperhidrosis Reduction: of palms & axillae using Botulinum toxin injection.

Pilonidal Sinus of natal cleft management: including Incision of abscess, exploration and laying open of sinus tracts with healing by secondary intention or Reconstruction with skin graft, local or regional flap.

Rhinophyma Reduction: by formal shaving/ paring with CO2 laser.

Skin & Soft tissues Tumours Management:

Biopsy: using various techniques including FNA, Incision / excision or Trucut biopsy.

Benign lesions: Curette, shave & excision with direct closure or reconstruction using skin graft or various flaps.

Malignant lesions including BCC, SCC, MM, Fibroxanthoma, Pheochromocytoma, fibrosarcoma/ DFSP, angiosarcoma, desmoid tumour etc. Wide local excision or compartectomy with direct closure or Reconstruction using skin graft, local / regional, distant flaps.  Free flaps.

Amputation: Digital amputation at different levels, Ray /Forefoot amputation, Above & below Knee amputation.

Lymph node surgery:  Open & Sentinel biopsy Neck, Axilla /supratrochlear, Groin/ pelvis and Lymphadenectomy including axillary & groin. First assistant in Ilioinguinal/ popliteal & Neck Dissections.

Adjuvant Therapy: Electrochemotheraphy for in transit/ metastatic melanoma deposits & hypertrophic scars and Co2 Laser ablation for in transit metastatic deposits.

                                                                      

Burns Management:-

 

Acute burn management:

Acute Burn Release: Escharotomy for full thickness burn: including Fasciotomy of upper/lower limbs & release of trunk.

Vascular Access: including venous cut down & interosseous access placement.

Acute Burn Resuscitation: Resuscitation of major burns in Adult/ child (10% to >50%)

Burn debridement & dressing: including Tangential or formal excision of burnt skin/ tissue (< 5% to >50%) involving trunk, limbs & special areas eg. hands, face.

Post debridement resurfacing: with SSG /FTSG / Flaps (local, regional & distant), Cadaver skin & skin substitute including  integra.

Late burn surgery:

Post burn contractures release: using different techniques including simple transverse,   z-plasty, w-plasty, VtoY & fish mouth to formal excision.

Post release defect resurfacing: with SSG, FTSG, skin substitute eg. Integra & SSG and Flaps (local, regional & distant).

 

Trunk & Urogenital:-

 

Chest (excluding breast):

Congenital: eg. Poland syndrome deformity correction using expander /implant &/or Latissimus Dorsi flap and in cases of Spina bifida soft tissue cover using skin graft, local advancement / rotation flaps.

Sternal dehiscence / defect (post resection) closure: including using Vac application, unilateral /bilateral pectoralis major flap with or without division of its insertion.

Chest wall defect reconstruction: using regional flaps eg. Scapular / para-scapular or pedicled flaps eg. Pectoralis major & Latissimus Dorsi.

Abdominal wall:

 Defect (post resection) Reconstruction /Hernia (Umbilical) repair: using Vac dressing / SSG, local /regional flaps eg. Rectus Abdominus musculo-cutaneous flap, Groin flap & application of prosthetic material eg. mesh.

Pelvis/ Perineum:

Congenital; circumcision (for recurrent balanitis, trauma & religious), Horton’s test / ventral penile chordae release, hypospadias repair (single /two staged reconstruction).

Reconstruction; Perineal / buttock, Penis/ Scrotum defects using skin graft, local / regional flaps.

 

Breast:-

 

Mastectomy: performed subcutaneous bilateral mastectomies for gender reassignment and minor excisions including lumpectomy.

Reconstruction:  performed/ taught delayed unilateral /bilateral breasts reconstruction using sub-pectoral breast expander/ prosthesis, Latissimus Dorsi musculo-cutaneous flaps with breast expander /implants plus nipple reconstruction and assisted in reconstruction using TRAM (pedicle/free), Free DIEP/ SIEA flaps.

Reduction: performed/ taught unilateral & bilateral breast reduction using different techniques including Central mound/ amputation with free nipple grafts, Supero-medial pedicle, Inferior pedicle, Wise pattern and vertical scar plus revision surgery.

Mastopexy: performed unilateral & bilateral breasts uplift using different techniques including Circum-areolar, Wise pattern & Vertical scar /Millard techniques with or without Liposuction /  breast implans (sub-glandular or sub-muscular).

Augmentation: performed unilateral & bilateral breasts augmentation using different techniques namely subglandular or submuscular and various types of implants/ prosthesis or expanders plus removal /replacement of expander/ prosthesis with or without capsulorrhaphy, capsulotomy or capsulectomy.

Gynaecomastia: performed unilateral or bilateral gynaecomastia excision via peri-aerolar incision and gynaecomastia reduction with liposuction or combination of both above techniques.

Nipple/areolar complex:  performed; simple excision of accessory nipple, correction of inverted nipple, nipple/ areolar reduction, neo nipple reconstruction with local Fish tail or Trifoliate flap and neo-areola reconstruction with FTSG.

Miscellaneous: performed procedures including removal of tissue expander port, re-adjustment of reconstructed breast, re-construction of  Inframammary fold, division of thoraco-dorsal nerve post reconstruction with Latissimus dorsi flap.

 

 Lower Limb:-          

 

Reconstruction:  

Soft tissue: performed various procedures to cover defects ranging from simple Skin graft to various flaps including local flap eg. Rhomboid, transposition, rotation, island/keystone flaps etc. and regional flaps eg. Groin, Tensor fascia lata, gracilis, Gastrocnemius, sural artery etc. Assisted in Free flaps eg. Fibular, Latissimus dorsi flap.

Tendons, nerves & vessels: performed exploration and repair of various nerves, tendons & vessels plus harvesting of sural nerve, plantaris tendon, and saphenous vein.

Bone: performed MUA & fracture fixation of small bones of foot egg. K-wiring /plating of metacarpals & phalanges plus harvesting of iliac bone graft.

Amputation:   performed following namely above knee /below knee, fore-foot, trans-metatarsal /ray amputations and amputations with fillet flap.

 

Upper Limb:-

 

Tumour extirpation/ Dupuytyrens:

Tumour; excision of dorsal wrist ganglion, flexor sheath ganglion/ myxoid cyst/ giant cell tumour, Synovectomy.

Dupuytyrens disease: performed procedures including simple fasciotomy / selective fasciectomy / dermofasciectomy (single or multiple digits /palm) and been involved in collagenase injection followed by manipulation.

Decompression: steroid injection / surgical release of tennis elbow +/- ulnar nerve transposition,  De Quervain’s tenosynovitis, carpal tunnel syndrome / guyon’s canal & trigger finger plus  septic joint /tendon washout and tenolysis.

Reconstruction :

Tendons: including primary repair of extensor / flexor tendons in all zones (multiple / single), delayed repair /lengthening (single stage with palmaris longus graft or two staged using silicone rod / tendon graft). Tendons cross hitching/ transfer eg. EI to PL & FDS to FDS plus using Mitek mini anchor for open mallet & central slip injuries.

Ligaments: including simple repair to reconstruction (eg.using palmaris longus/Mitek) of ligaments / volar plate of digits (eg UCL of finger / Thumb) to ligaments of wrist.

 Bones & Joints: performed procedures ranging from EUA/MUA to ORIF of various types of fractures (eg.transverse, oblique/ spiral, communicated) of hand bones ( carpal, metacarpals, phalanges) using various techniques including splintage, external fixators (dynamic/static), K-wires, cerclage wires, plating (static, dynamic, condylar) and removal of these hardware. Also used above techniques for small joints, performed various osteotomies (eg.rotation, wedge/ angulation) and arthrodesis with or without bone graft. Beside have assisted in procedures ranging from arthroplasty (eg.using FCR tendon, silicone/ metal), small joints replacement to various operations for Rheumatoid hand deformities eg. Boutonniere,  swan neck & ulnar drift etc.

Revascularization /Re-implantation: performed revascularization of single digit and repair of arteries at elbow & wrist level plus re-implantation of finger at proximal / middle phalangeal level. Also been involved in pollicisation.

Amputation: performed following including digital amputation at different levels /ray amputation and amputation with fillet flap.

Congenital:  performed excision of accessory digits in cases of Polydactyly, excision of extra thumb in case of Duplicate Thumb and various procedures (FTSG / local flaps eg.zig-zag, dorsal rectangular etc) for different types of Syndactyly (ie.simple,compound /complex and incomplete / complete).Also been involved in surgery for radial club hand.

Soft tissue cover: performed using SSG/FTSG, Local flaps (eg. Atasoy, Kutler, Venkatswami advancement, Hatchet, bilobed in radial club hand  etc.), Regional flaps. Cross finger dermal /reverse dermal, Thenar, Neurovascular digital island, Foucher/1st MCA, Reverse Radial forearm, reverse Ulnar artery etc.) And Distant flaps. Groin, chest).

 

Auricular:

 

Reconstruction:   performed using FTSG flaps egg. bilateral  helical advancement, trap door,  post auricular tube pedicel, lobular repair /reconstruction for split ear lobe & after  radical excision of keloids and have been involved in ossteointegrated implant procedure and re-implantation of near total avulsed ear.

Congenital:  performed simple excision of accessory skin tags/auricle, correction partial cryptocia, correction of prominent ears using various techniques (eg. cartilage incision/ scoring with concho-helical/ mastoid & mustarde sutures). Assisted in all stages of complete reconstruction of neo ear using rib graft.

Cleft Surgery:

 

Cleft lip: performed primary incomplete / complete Lip repair (Unilateral /Bilateral) using Millard  technique.

Cleft palate: performed primary cleft repair of hard / soft palate using Von-Langenbeck

technique with or without Vomer flaps and assisted in Wardill-Kilner & Furlow’s techniques.

Cleft Rhinoplasty: performed / assisted in closed nasal tip plasty using Tajima sutures / open rhinoplasty with columellar lengthening.

Other procedures: performed / assisted in Alveolar iliac bone havesting /grafting, palatal fistula repair using Tongue flap, Orticochea pharyngoplasty and minor / major revesion of lip / palate.

 

 Craniofacial Surgery:

 

 Bony trauma: performed MUA of nasal bone fracture, Gilles elevation of zygomatic fractures, Plating of infraorbital fracture and assisted in intermaxillary fixation / mandibular plating.

Soft tissue: performed excision of facial dermoids (external approach), debulking of facial haemangiomas and lipofilling for facial contour augmentation.

Miscellaneous: performed procedures for facial palsy like gold weight insertion upper eyelid, tensor fascia lata static sling, canthpexy / tarsorrhaphy and application of frost stitch plus, like Ptosis correction using fascia lata. Assisted in mandibular external distraction for hypoplasia.

 

Head & Neck:

 

Scalp: have performed burring of calvarium, minor/ major resection of scalp and reconstruction of defects with SSG/FTSG, Local flaps (eg. transposition, rotation, hatchet/ skew, rhomboid,

Ear: have performed wedge excision, partial/ total pinnectomy and reconstruction with SSG/FTSG or Flaps eg. Helical advancement, trap door & staged post-auricular tube pedicle flap.

Eye: performed wedge excision of upper/ lower eyelid up to  1/4th  with direct closure, SSG/FTSG for superficial defects and flap reconstruction for full thickness defects of lower eyelid more than 1/3rd  of width eg. Mustarde / Mc Gregor  &  flap from other eyelid etc.

Lip: performed V/W wedge excision upto 1/3rd with direct closure and flap reconstruction for more than 1/3rd defect eg. Abbe, Abbe-Estlander, Karapandzic

and for superficial defects / lip shave SSG/FTSG or mucosal advancement flap.

Nose: performed superficial, deep & full thickness excisions (partial rhinnectomy) and reconstruction using FTSG, composite graft (chondro-cutaneous), local/regional Flaps eg. bilobed, nasolabial, cresentric advancement, forehead / glabellar etc.

Neck & Salivary: Performed Sentinel / open Lymph node biopsies,  partial parotidectomy for adenoma, excisions and reconstruction with FTSG/SSG, local / regional flap including Pectoralis major & Latissimus dorsi musculo-cutaneous flaps etc. Have assisted in various types of Neck dissections, superficial / total parotidectomy and reconstruction with free flap including RFF, Jejunal free flap, fibular etc.

Facial reanimation: Performed Gold weight insertion, Fascial lata sling.

 

Aesthetic  

 

Face & Neck:

 Face/ Neck lifting: Performed Bicoronal / hairline brow lift, Midface lift with Endotine , Submental liposuction/ lipectomy and full face/neck lift with SMAS plication.

Eyelids: Performed lower Blepharoplasty via subciliary incision with or without fat pads resection, upper blepharoplasty with or without fat pads resection, lateral tarsorrhaphy, and canthopexy / canthoplasty and epcanthal fold correction.

Nose: Performed Rhinoplasty (open/closed) with or without osteotomies, open Tip plasty with or without cartilage graft, silicone nasal implant.

Hair Transplant: performed FUT including strip harvesting, recipient site preparation with grafts insertion.                                                               

Ears: performed Prominent ear correction with or without cartilage scoring, mustarde / concho-mastoid sutures, Loop ear correction, redo pinnaplasty and split ear lobe repair. Chin: performed chin augmentation using silicone implant.

Facial rejuvenation / countering:  using Botulinum toxin, chemical peel, Laser peel / resurfacing and dermabrasion and facial contouring using fat or dermal graft & various facial fillers.

Trunk:

Abdomen: Performed various types of abdominoplasties (eg. Mini, full with umbilical transposition & recti plication, Apronectomy in dermolipectomy fashion, Fleur de lys.

Umbilicoplasty & adjunctive Liposuction/ Liposculpture.

Buttock: Performed open Buttock uplift and Liposculpturing.

Genitalia: Performed Labia minora surgical reduction & Penile lengthening with division of suspensory ligament).

Limbs:

Upper Limb: performed open upper arm reduction and Liposculpturing.

Lower Limb: performed open Thigh reduction & Liposuction.

Laser:

Tuneable Dye: used it for portwine stain.

NdYag: used for spider nevi, telangiectasia and thread veins.

CO2: used for facial resurfacing.

                 LEADERSHIP QUALITIES 

        I consider my self  member of a team but when the situation demands, having appropriate experience, I  have the capability and ability to lead the team. I was  Heading the Plastic Surgery Section in Aga Khan University Hospital, Karachi, Pakistan.

                  I believe in quality work but realise the importance of time management both in my job and personal life.

 

                                                     

                                         CAREER PLANS

 

       I intend to pursue my career in Aesthetic / Plastic & Reconstructive Surgery, having  worked in this speciality for  more than 17yrs during which I have been exposed to almost all aspects of patient care.

 

        I have worked in this dynamic speciality for 12 years in 4 reputable hospitals of Ireland and 4 hospitals of United Kingdom, the last being Associate Specialist in Plastic Surgery having independent clinics and operative lists.

      

         I decided to  move to Pakistan in 2007 after getting job offer as Assistant Professor  Plastic Surgery in Aga Khan University Hospital, Karachi which gave me significant experience in heading  a Plastic Surgery unit in one the most reputable hospital of Pakistan though I was not allowed private practise being a full-time faculty.

        

        After working for last 5 years in this hospital I have decided to go into private practise on my own as a Consultant Plastic & Hand Surgeon since June 2012. At the same time I have joined as a part time faculty Ziauddin University Hospital, Karachi which is another very modern Institute of Pakistan, with multiple campuses providing quality health care for most of the regions of this huge metropolitan city. 

 

I moved back to United Kingdom in September 2013 after being offered the position of Associate Specialist in Plastic & Reconstructive Surgery in the prestigious St. George’s University Hospital, London.

        

         I am very confident in dealing with the routine Plastic surgery cases though my special fields of interest include Hand Surgery, Burns & Aesthetic surgery.

 

         I intend to be part of an organisation / hospital which excel in their services and patient care.

Sports:  I am a keen sportsman interested in playing cricket as an off spin baller and play table tennis regularly.

 

Painting: I am an artist and have been painting both in oil and water colour since childhood but presently practising sculpturing.
